    MyChart is a communication resource used by the DAP care team to communicate with patients over a secure, confidential platform. MyChart can be accessed via the DAP website. To register, visit the MyChart page, click “Sign Up Now” and enter your Activation Code as it appears on your enrollment letter (your code is not case sensitive).

    What happened to MyChart?

    If your keyboard remains idle for 15 minutes or more while you logged into MyChart, you will be logged out of MyChart automatically. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.
